A Structural Model for Predicting Sexual Satisfaction Based on Fetal Attachment and Body Image Flexibility, Mediated by Pregnancy Anxiety, Among Pregnant Women in Sirjan

Abstract

The aim of this study was to predict sexual satisfaction based on fetal attachment and body image flexibility, with pregnancy anxiety acting as a mediator, among pregnant women in Sirjan. This study is applied in nature and employs a descriptive-correlational design. The statistical population consisted of all pregnant women residing in Sirjan during the spring of 2026; from this group, 262 participants were selected using a convenience sampling method. The research instruments included the Larson Sexual Satisfaction Questionnaire (1998), the Cranley Maternal-Fetal Attachment Scale (1981), the Sandoz Body Image Flexibility Questionnaire (2013), and the Huizink Pregnancy-Related Anxiety Questionnaire (2004). Data analysis was conducted using Pearson correlation coefficients and structural equation modeling (SEM), utilizing SPSS (version 27) and AMOS (version 24) software. The results indicated that sexual satisfaction among pregnant women in Sirjan can be predicted by fetal attachment (β = 0.240, P = 0.01) and body image flexibility (β = 0.225, P = 0.01). Furthermore, sexual satisfaction was predicted by fetal attachment (β = 0.097, P = 0.01) and body image flexibility (β = 0.176, P = 0.01) with pregnancy anxiety acting as a mediator. Research findings indicate that maternal-fetal attachment and body image flexibility—by reducing pregnancy-related anxiety—create a favorable foundation for improving the quality of marital relationships and enhancing sexual satisfaction. Accordingly, it is recommended that intervention packages aimed at strengthening the maternal-fetal emotional bond and fostering a positive body image be developed and implemented within the educational and counseling protocols of health centers.
